SeaStar Senior Services is a new senior care company in Portland, Oregon. We provide in home care, companionship, and care management for seniors and vulnerable adults in Portland and the surrounding towns. We are looking for professional care managers who want to be part of a new company that is focused on providing quality, compassionate care to our clients.

We're currently looking for a part time care manager in the Portland metro area. Hours are minimal to start, 5 to 10 hours a week, but as the company grows, so will your hours.

Duties:

*Assess, plan, and coordinate services needed by your assigned clients.

*Update assessments and care plans quarterly or as needed.
*Respond in timely manner to changes in client's condition or environment
*Determine housing needs and assist in placement or obtaining resources for client safety
*Attend care conferences and serve as liaison between facilities, family, and professional fiduciaries
*Coordinate all medical appointments, specialty follow up visits, procedures, and obtain needed medications.
*Obtain other needed professional services including legal, financial, care, and more.
*Provide or arrange for transportation to appointments and social outings
*Document all work performed on client's behalf with detailed notes and accurate time
*Perform other related duties as assigned

Qualifications:
*Experience working with seniors and adults with disabilities
*Excellent driving record
*CPR certified or able to obtain
*Good observational and writing skills
*Must have excellent listening skills with critical thinking
*Computer skills required
*Master's in Social Work or RN required
*Must be able to pass a background check and drug screening

Compensation:

*50.00 per hour DOE

Job Types: Part-time, Per diem

Expected hours: 5 – 10 per week, will increase

Evening on call rotation
