Description
The Peer Support Specialist will be an essential part of the Behavioral Health Team. Provides telephonic and in person support and assistance with connection to outpatient behavioral health, substance use and other community services to patients who have recently discharged from the hospital. The peer support specialist may work remotely, in the hospital, clinic, in the community, the patient's home and/or telephonically or wherever the patient will need support. Connects people served with available community services while promoting the use of natural supports and existing community relationships.
