As Nurse Practitioner, you are going to be a key member of the Patient Sortal Health team and will share responsibility for the quality of care Patient Sortal members receive, the efficiency of which care is provided, and the development of what could be the largest Primary Care system for justice involved individuals.
· We are looking for a Licensed APRN Nurse Practitioner with a passion for criminal justice reform and social impact.
· Our healthcare system will have a focus on providing In person and telehealth consultations, medical history, screenings, physical exams, order and review labs and imaging to individuals returning back to our communities from correctional facilities.
About Patient Sortal:
Patient Sortal is bridging the gap of care for individuals being released from correctional facilities that have ongoing healthcare needs. We represent returning citizens upon release and have all their healthcare data necessary to provide high quality continuity of care. We will be their primary care medical home when returning to their communities.
Required
Active and unrestricted APRN License in PA
Minimum 4 year practice experience as a nurse practitioner
Valid NPI number and updated CAQH profile
Active DEA registration
State controlled substance licensure for DE (not required in PA)
Proficient in using electronic health records
Must own a computer (desktop or laptop) with a webcam and high-speed internet
Comfortable working remotely/autonomously
Excellent communication and collaboration skills
Must be able to work full time, 40 hours per week
No prior disciplinary action from a state medical board
Ideal Candidate
5 years of nurse practitioner experience in a primary care setting
Patient-centric and service-oriented approach to medicine
Prior experience working in telemedicine/performing virtual visits
History of working in a federally qualified health center, correctional health setting, or with other medically underserved communities is a plus
Prior leadership and/or managerial experience in a healthcare setting
