Job Summary: Brown’s Harbor Success Coach will be responsible for providing one-to-one coaching and guidance to the young adults living at Brown’s Harbor.
Responsibilities: Follow Children’s Harbor/Brown’s Harbor policy and procedures. Adhere to ChildNet, the Dept. of Children & Families, and BBHC relevant agency rules, regulations, and contractual guidelines. Meet weekly with each young adult assigned to the caseload (meeting annual contractual guidelines established by BBHC). Maintain case files and enter progress notes (BBHC portal) within 24 hours Maintain CQI (Track and Trend) – intake, discharge, progress notes, etc. Work with the teen’s transitional living advocate or IL Specialist/assigned case manager to ensure access to all available resources, benefits, and information regarding transition planning is available to the young adult. Assist youth with TIL housing transition, such as accompanying youth to housing interviews, completing applications, and aiding with move-in. Provide oversight to keep the community center and computer lab open during office hours. Meet with the BH Director bi-weekly for supervision. Promote a culturally positive workplace regarding the team and clients. Attends and participates in staff development opportunities. Adhere to any additional duties assigned by BH director, Director of Operations or CEO. |
