The clinical pharmacist assesses medication appropriateness and effectiveness for hospitalized patients, and collaborates with healthcare practitioners regarding interventions necessary to optimize patient care. The candidate must possess knowledge regarding standard concepts, practices, and procedures within the field of clinical pharmacy, and have the ability to effectively communicate their thoughts and recommendations.The clinical pharmacist will be required to operate in both a centralized and decentralized fashion. Required Central Operation Duties:
Process medication orders and communicate pertinent drug interactions, adverse reactions, allergy concerns, and contraindications to the healthcare provider
Verify medications for dispensing
Prepare IV admixtures using aseptic technique
Review and prepare chemotherapeutic regimens
Oversee support staff and effectively triage the dynamic workload to meet patient needs
Maintain availability for on-call support
Ultimately ensure timely, accurate dispensing of medications.
Required Clinical Duties:
Perform pharmacokinetic analysis and workup to optimize medication therapy based on evidence based literature
Manage TPN therapy to meet dietary recommendations, and ensure overall patient safety
Manage anticoagulation therapy
Perform renal dose adjustments
Evaluate therapies for IV to PO interchange
Perform therapeutic interchanges
Understand current clinical initiatives and intervene as needed to maintain compliance
Perform patient education and medication reconciliation
Participate in code blue response and maintain BLS, ACLS, and PALS certifications
Attend and actively participate in multidisciplinary rounds, clinical meetings, and committees
Perform antimicrobial stewardship activities, including profile and culture review, in order to make sound antibiotic interventions that are programmatically aligned with the recommendations and pathways of the ASP committee.
Provide staff education
The candidate must have an active AK pharmacist license in good standing or have the ability to reciprocate licensure.A Pharm D is required with residency training and/or BCPS preferred, and at least one year of post graduate hospital experience required.
