Starting Rate: $21.00 per hour
SUMMARY OF DUTIES:
As a Personalized Recovery Oriented Services team member, you will work collaboratively in a dynamic, person-centered, evidence-based program. This position works with adults diagnosed with a serious mental illness in a broad range of integrated services designed to help the client achieve goals of work, education, relationships, socialization, living and wellness. Strong judgment, problem solving and decision making skills along with interpersonal and organizational skills are essential for the position. The candidate must be capable of conducting individual interventions in PROS service areas including: wellness self-management, relapse prevention, basic living skills, family psycho-education and integrated dual disorder treatment.
RESPONSIBILITIES:
· Under supervision, provides direct services to client recipients including individual and group sessions.
· Developing and scheduling PROS treatment activity programs.
· Conducts comprehensive behavioral health assessments.
· Routinely conducts lethality and mental status assessments.
· Provides crisis intervention as needed.
· Responsible for the management of client charts.
· Develops services plans.
· Documents consumer progress as per established guidelines.
· Able to communicate with others in the client services system to coordinate care.
· Provides program consultation and education to community members.
· Participate in case review team meetings, conferences and in-services trainings.
· Collaborate with care managers and other clinical staff.
· Utilize the Electronic Medical Record system.
· Schedule and assigned duties are subject to change based on the needs of the program and the clients we serve.
QUALIFICATIONS:
· Bachelors degree in psychology, social work, recreational therapy, human services or other closely related mental health field
· Supervised field practicum in any organized mental disability or rehabilitation setting with one year (1) full time paid direct service experience in an organized mental disability or rehabilitation program
OR
· Associates degree in human services with four (4) years full time paid direct service experience in an organized rehabilitation program two of which must have been in mental health.
