Job Title: Nurse Practitioner – Risk Adjustment

Location: Multiple Clinics in Orange County. Travel between Huntington Beach, Costa Mesa, Newport Beach, Irvine, San Clemente, Laguna Beach, Tustin. No more the 2 clinics per day.

Responsibilities:
Provide multidisciplinary patient care from admission through discharge.
Conduct history and physical exams, diagnose, and treat medical problems.
Order and interpret diagnostic and laboratory tests.
Perform minor procedures and facilitate the discharge process.
Identify and execute Annual Wellness Visits (AWV) and Annual Medicare Advantage Visits (AMAV) for Medicare patients.
Manage care for patients with chronic illnesses, particularly in the adult and geriatric populations.
Demonstrate advanced knowledge in Risk Adjustment programs and HCC coding and documentation guidelines.

Qualifications:
Required:
Master’s degree in Nursing.
Advanced clinical knowledge and skills in patient care and management.
Proficiency in Microsoft Office products.
Current RN/NP license in California, furnishing license, and DEA number.
BLS and ACLS certification.
National board certification as a Nurse Practitioner in the area of specialty.
Preferred:
Two years of recent clinical experience as a Nurse Practitioner in ambulatory and acute healthcare settings.
Bilingual or multilingual candidates.
