Job Overview:
We are seeking a detail-oriented Clinical Coordinator to join our healthcare team. The Clinical Coordinator will play a vital role in ensuring effective and efficient coordination of patient care, and supporting healthcare providers.

Duties:
- Coordinate patient appointments, admissions, and discharges
- Manage patient records and ensure accuracy and confidentiality
- Assist healthcare providers with procedures and examinations
- Communicate with patients to provide information and follow-up care instructions
- Collaborate with medical staff to optimize patient care delivery
- Oversee clinic inventory and supplies management
- Maintain a clean and organized clinic environment

Skills:

-REQUIRED: Licensed Practical Nurse or Certified Medical Assistant
-Strong knowledge of medical terminology and medications

-Professional and engaging communicator

-Familiarity with anatomy
-Ability to focus - providing accurate and through documentation
-Experience working with diverse patient populations including geriatrics
-Previous experience with Coordination of Care (CCM) and Transfer of Care (TCM) programs preferred.
