Due to growth we are hiring a Home Health RN to join our team!
Requirements:
- Current CA RN, BLS hands-on CPR certification
- Valid driver’s license and auto insurance
- Bachelor's degree preferred, with one to three years of home health care experience

Travel – This is a traveling position. Must maintain a valid driver’s license, active auto insurance meeting state minimum requirements and an operational vehicle.
Duties:
- Completes an initial assessment of patient and family to determine home care needs. Provides a complete physical assessment and history of current and previous illness(es)
- Regularly re-evaluates patient nursing needs
- Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient
