We need a Medical Records Coordinator to join our team at Sycamorespring Skilled Nursing and Rehabilitation in Miamisburg, OH .
You’ll maintain medical records according to State and Federal regulations to ensure their timely and accurate completion.
$13.50 to $18.50 Per Hour You’ll Be Responsible For : Performing routine audits of in-house charts quarterly & upon admission to ensure completeness & accuracy Completing focus & PI audits on specific topics as directed by Administrator or Regional Medical Records Director Notifying staff of deficiencies found in the record & following up to ensure the in-house records are complete Assembling the medical records of discharged residents Analyzing discharged charts for completeness & following up with staff / physicians to ensure they are complete Processing requests for medical information as directed by the Administrator, Regional Medical Records Director, or corporate Risk Manager (i.
e., requests from residents, attorneys, for insurance reimbursement, in response to a subpoena). Maintaining Release of Information Log for all medical records requests, copies released for payment, continuity of care, & health care operations Completing Accounting of Disclosures form according to HIPAA policies & filing it in resident’s chart Maintaining the confidentiality of medical records & health information Monitoring physician visits & reporting non-compliance to the DON / Administrator at least monthly Purging charts, maintaining destruction log, & maintaining information for records at off-site storage facility Assigning ICD-9 CM codes for resident diagnoses Entering diagnoses & codes in HCS system Coordinating physician dictation with outside transcription company Filing any loose sheets needing to be attached to the record Maintaining supply of forms in department & on nursing units Completing discharge / transfer notice when necessary Entering discharge / transfer data in HCS system and ECS Forwarding requested records to Carespring Case Manager for managed care updates Serving as facility contact person to oversee Record Retention & Destruction Policy working with each department as documents / records are boxed for long term storage Signing off on each Record Log to confirm proper completion You’ll Need To Have : Medical Records Certification (or be willing to obtain it) Strong attention to detail & the flexibility necessary to work in a fast-paced environment PointClickCare experience (preferred) Knowledge of Microsoft Word & Excel We’re Proud To Offer You : Paid Time Off Paid Training Health Insurance Dental Insurance Vision Insurance Life Insurance Disability Insurance 401K with Employer Match Flexible Schedule Education Scholarships Continuing Education Credits Employee Discounts Exceptional Care & Quality Service are the hallmarks of Sycamorespring's in-house team of talented and experienced licensed therapists, nurses and caregivers .
