*CareOne at Livingston NJ (Acute Rehab Skilled Nursing Facilities) - Hiring for MDS Coordinator to join our team*

Responsibilities:
- Provide direct patient care, including assessment, planning, implementation, and evaluation of patient care needs
- Collaborate with healthcare team members to develop and implement individualized care plans
- Administer medications and treatments as prescribed by the physician
- Monitor and document patient's progress and respond to any changes in condition
- Educate patients and their families on healthcare procedures, medications, and self-care techniques
- Utilize electronic health record systems such as Athenahealth to maintain accurate and up-to-date patient records
- Participate in utilization review activities to ensure appropriate use of healthcare resources
- Provide medical management support to patients with chronic conditions
- Stay updated on current medical guidelines and best practices

Requirements:
- Valid nursing license in the state of [NJ or Multi-State]
- Minimum [3] years of experience in acute care or similar setting preferred not required
- Strong knowledge of Medicare guidelines and regulations
- Familiarity with utilization review processes and medical management principles
- Proficiency in electronic health record systems, such as Athenahealth
- Excellent communication and interpersonal skills to effectively interact with patients, families, and healthcare team members
- Ability to prioritize tasks and manage time effectively in a fast-paced environment
