We have an immediate opening for a Clinical Liaison who can build relationships and support care coordination for our community’s most vulnerable.

Qualifications and Requirements:
- Must hold an LPN or RN license in good standing
- Positive attitude, great communication skills, and ability to work independently
- A solid personal character with an empathetic personality
- Leverage current client relationships and establish new partnerships with physicians, hospitals, clinics and rehab facilities
- Must be comfortable collaborating with referral sources to generate new business
