Responsibilities:
The ideal candidate will serve as the designated Patient Care Navigator to an assigned group of patients. The Patient Care Navigator will partner with the patient and care team to support patients' health-related goals through monthly contact. Patient contact may occur by phone, electronic communication, and on-site interactions. Greater than 50% of time will be spent making outbound patient calls. Work may be on-site, hybrid, or fully remote as required to meet client needs.
