About this opportunity :

• Works with Family First and Pathway of Hope families and their case managers to support youth educational goals. Engage, accept, and provide comprehensive, structured, complex, character-building services for youth participating in the Family First and Pathway of Hope programs;
• understand the uniqueness of the individual’s history to determine the most effective program plans; develop comprehensive program plan / goals and evaluate youths progress by conducting mentoring and coaching sessions with parents, guardians, and staff;

serves as an advocate for youth to acquire services that will enable them to cope with their environment functionally; ensures continued compliance with funding requirements.

Travel to other counties in the service area as needed to conduct case management and school visits

What we are looking for in you :

• Bachelor's degree from an accredited college or university in Social Work, Behavioral Science, or a related field,
• Three years of progressively responsible experience providing direct case management social services, including accessing youth needs and developing individual, comprehensive, long-term action plans for recovery utilizing a wide variety of resources,
• Any equivalent combination of training and experience which provides the required knowledge, skills, and abilities.

Physical Requirements and Working Conditions :

Ability to meet attendance requirements.

Ability to read, write, and communicate the English language effectively.

Ability to communicate clearly and effectively both orally and in writing.

Ability to think clearly and quickly to maintain control of client caseload.

Ability to work on multiple cases effectively and efficiently simultaneously without becoming frustrated or disorganized.

Work requires the ability to lift, carry, and push / pull light (less than 25 lbs.) materials, supplies, and equipment. Light mobility : duties are usually performed by standing, walking, and primarily sitting.
