Northwest Home Health & Rehab is Hiring a Director of Nursing Near Hills, IL

Northwest Home Health & Rehab is an independent, family owned, Medicare-certified home health agency that has been in business since 2004. We are known by our patients, community and referral sources as providing the highest quality of care. We are looking to hire an experienced Director of Nursing to join our team!

As the Director of Nursing (DON) with Northwest Home Health & Rehab, you’ll enjoy the autonomy in a family owned business along with highly competitive pay as well as a generous signing bonus.

Office Location: Lake in the Hills

Hours: 8AM- 5PM Monday through Friday

Responsibilities:

  • Reporting directly to the Administrator, this registered nurse (RN) position is responsible for overseeing the quality of patient care
  • Supervise the home health field staff including registered nurses, physical, occupational and speech therapists and certified nursing assistants
  • Ensures patient plans of care are developed, implemented and updated
  • Manages the triage process
  • Ensure compliance to Medicare and IDPH regulations
  • Displays a willingness to support agency policies and procedures and participates in the change and updates of such policies
  • Conducts patient case conferences, field staff in-services and/or staff meetings and maintains documentation
  • Supports the recruiting and orientation of new hires
  • Shared on call commitments
  • Lead the efforts of maintaining a high star rating
  • Oversees Agency’s ongoing Quality Assurance and Performance Improvement (QAPI) program
  • Attends pertinent continuing education programs other than routine in-services and shares information with staff as needed
  • Participates in survey preparation
  • Other duties as assigned

Qualifications:

  • Registered Nurse licensed in the state of Illinois
  • 2 years of home health supervisory nursing experience required
  • Knowledge of state and federal home health guidelines
  • Knowledge of home health documentation
  • Experience with electronic medical records (Axxess)
