Relief Home Health Services is Hiring a Scheduling Coordinator Near Hyannis, MA

Make a Difference in your community! Relief is looking for a Scheduling Coordinator to join our Hyannis based team.
Relief Home Health Services, Inc. primarily serves patients with multiple co-morbidities including but not limited to: substance abuse, behavioral health, homelessness, and complex medical diagnoses. We are looking to expand! You will work with a comprehensive team, including collaborative community resources, whose goal is to reduce hospital utilization and improve patient outcomes. We’re looking for an individual who has a friendly demeanor and is passionate about supporting others, has a strong desire to analyze problems and identify solutions, has a proclivity for technology and enjoys mastering new systems.
The scheduling coordinator is responsible for managing the schedules of field staff on a daily basis, assigning patient visits according to region and team, managing ongoing patient needs in the community and working with clinical staff to identify inconsistencies related to patient care. The successful candidate has the ability to adapt to constantly changing priorities while maintaining high attention to detail and accuracy. The scheduling coordinator is crucial to the success of our continuity of care in the community.
Qualifications
  • A high level of computer literacy, technical proficiency and a desire for a computer-based job
  • Intermediate to Advanced skills with MS Excel and Google Workspace
  • Adept at understanding and mastering technical applications, programs and processes
  • Proactive, Self-motivated, able to meet strict deadlines
  • Propensity for analytical thinking, identifying patterns and proposing solutions
  • Able to maintain composure in a deadline-intensive environment
  • Strong propensity for working with and helping people
  • Process-focused organization style with keen attention to detail and an eye for proactively identifying potential problems
  • Desire to collaborate with others and model positivity
  • A minimum of two (2) years of recent experience in a home care agency or other healthcare field is preferred.
Duties/Responsibilities
  • Schedule all incoming patients within 48 hours of receiving referral
  • Determine patient needs and provide appropriate services in the community
  • Maintain nurses' schedules and workload for full-time employees
  • Cooperates with Scheduling Manager regarding investigation of patient feedback
  • Cooperates with Scheduling Manager regarding new processes developed and improves workflow and efficiency
  • Manage phone answering service and on-call schedules
  • Notifies the Agency of emergencies, sickness, and other imminent occurrences that may affect the patient caseload as quickly as possible relative to the events occurrence.
  • Coordinating coverage for staff time off.
  • All other tasks as assigned by manager
  • Roles may change due to Agency needs
