Health Care Coordinator
JOB SUMMARY:
HEALTH CARE COORDINATOR Full-time position for individual with ability to work without direct supervision. Responsible for coordination/scheduling of medical appointments for persons with developmental disabilities; communicating medical procedures and/or changes to other departments within the agency; maintaining medical information in proper formats and accompanying clients to medical appointments.
JOB REQUIREMENTS:
Must have valid driver's license with clean driving record; good oral and written communication skills. Proficiency in Outlook, Excel and Microsoft Word computer programs a plus. CNA not required, but preferred.

BENEFITS:
Upon successful completion of 90 days, all full time employees are eligible for health and dental insurance, Aflac products, and discount membership to Club 1 Fitness. After 6 months of employment, full time employees begin to accrue sick and vacation time at the rate of 1 vacation day per month and 3.33 sick hours per month. After 1 year of employment, full time employees become eligible to join the 401K plan.
