- Comply with regulatory agency standards and regulations and knows/adheres to all laws and regulations pertaining to patient health, safety and medical information.
- Evaluate new patients.
- Create and implement goal-oriented treatment plans. Establish goals that are functional, measurable and attainable.
- Treat patients and conduct re-evaluations to alter the plans of care and update the goals as individually required.
- Assists patient towards goal achievement through effective treatment plan and interventions. Develops home exercises and self treatment plans for the patient.
- Provides education to patient and family, including diagnosis and prognosis of therapy.
- Responsible for the accurate and thorough completion of, evaluation reports, progress notes/flow sheets discharge summary and any additional required documentation.
- Performs other duties consistent with purpose of job as directed.
- Report to the Clinical Manager as directed and to discuss and correct any potential patient care or documentation issues.
- Education and Licencing
- Successful completion of a Bachelor's Degree required.
- An active Physical Therapist license in the state of Indiana is required.
- Pelvic Health and Wellness experience preferred.
- Vestibular Rehabilitation and Balance Therapy experience preferred.
- EMR or "Electronic Medical Records" knowledge is preferred.
- Professional appearance and the ability to eloquently interact with patients and their family members.
