ESSENTIAL FUNCTION OF THE POSITION: The Social Service Coordinator assists in admission/discharge activities, provides ongoing social and behavioral adjustment interventions, completes documentation responsibilities as required/appropriate, and acts as an advocate for Residents and/or family members.

Job Types: Full-time

Minimum Education: Bachelor's Degree in Social Work, Psychology, Education, or related area of study

Minimum Experience: 2-4 years experience in social service related position. Extensive knowledge of: skilled care diagnoses, costs of care, reimbursement systems, Medicare and private insurances, Medicaid rules/regulations, IDPH mandates, etc.
