**Overview:**

We are seeking a dedicated Care Manager to join our team. The ideal candidate will have a strong background in healthcare and be passionate about providing quality patient care. Strong organizational skills and excellent attention to detail.

**Duties:**

- Coordinate patient care by assessing, planning, implementing, and evaluating patients' needs

- Collaborate with healthcare providers to ensure optimal patient outcomes

- Conduct comprehensive assessments of patients' health status

- Develop and implement care plans based on patients' needs

- Monitor and track patients' progress and update care plans as necessary

- Provide education to patients and families on healthcare management

**Requirements:**

- Prior experience in ICU, discharge planning, utilization management, or related fields

- Strong knowledge of medical terminology

- Familiarity with medical records and documentation practices

- Experience in acute care or primary care settings

- Ability to deliver high-quality patient care with compassion and empathy
