Position Purpose :

Provides individualized supports to individuals with intellectual disabilities to increase their ability to live independently in keeping with the overall goals and objectives of Fraser, Ltd.

to ensure quality implementation of services.

Fraser, Ltd Overview :

Established in 1893, Fraser Ltd., by charter is the oldest human service agency in North Dakota. Our mission is to provide support to children, youth and adults on their life’s journey to independence.

Fraser fosters the development of functional academic and independent living skills which are person centered and age appropriate.

A variety of residential services are available for individuals over the age of 18 who require subsidized housing. We employ caring, responsible, and compassionate individuals on our team of over 300 professionals.

Direct Support Professional Qualifications :

High school diploma or GED and at least 21 years of age. Sufficient training to work as a team member in providing a program that assists people to develop a sense of well being and skills that enable them to learn to function with greater self sufficiency.

Prefer a minimum of one year’s experience working with people with intellectual disabilities. Ability to relate to management, peers and people supported in a professional manner.

Time management skills and ability to organize workload. Competent in all ND Developmental Disabilities training modules within 18 months of employment.

Direct Support Professional Responsibilities :

• Assesses and assists to train people in accordance with the independent living curriculum and their Overall Service Plan (OSP) with priorities established by the Interdisciplinary Planning Team (IDT)
• Prepares meals and provides hands-on training to people in accordance with established menus
• Supervises medication and provides hands-on training in accordance with medication procedures
• Supervises behavioral areas as relevant to people living in the community and assists in personal adjustment to residential living
• Monitors health needs daily and follows designated procedures.
• Supports people with personal hygiene to achieve clean, well-groomed personal appearance throughout shift
• Completes assigned cleaning tasks and provides hands-on training to people
• Attends meetings for administrative, supervisory, and when designated, program planning meetings
• Reviews daily log notes, communication book entries, and the Medication Administration Records (MAR) at the beginning of each shift and records significant positive observations or concerns regarding the people supported each day
• Records daily census, number of meals served, sleep charts, kitchen documentation, and cleaning checklists
• Conducts fire drills in accordance with regulations and schedules, and completes necessary documentation
• Assists people in taking clothing and personal property inventories
• Records weights weekly in the weight chart
• Assists with yearly assessments
• Assists people in the community

Must have a valid driver’s license and pass a background check.
