About the Program:
Sunrise Services is recruiting for a Health Care Coordinator (HCC) to perform health care case management for individuals with developmental disabilities, and those with chronic mental illness. The HCC locates healthcare providers and specialists as needed, attends medical appointments, provides advocacy and transportation for participants, and ensures documentation of all health care contacts during and in between appointments.
The successful candidate will have basic healthcare knowledge, experience working with adults and/or children with developmental disabilities and/or mental illness, basic computer and office skills, exercise sound judgment in challenging dynamic situations and be able to read, assimilate and act upon multifaceted information. We are looking for someone who is reliable, prompt and always a professional.
Essential Functions:
- Assess all health needs; formulate and implement a plan of care; and re-evaluate the care plan as needed.
- Regularly observe clients for changes in physical, mental and emotional functioning to see if needs are being met by maintaining overall familiarity with the individual.
- Provide guidance, assistance for clients to be aware of need for health services and instruct, assist and consult with Program Manager, Direct Care Supervisor and other client support staff in observing, recognizing, and reporting health needs of clients.
- Notify Director, Program Manager or RN Supervisor if significant changes in medical conditions occur which may potentially require additional support services.
- Provide instruction to clients and staff for the use of any and all prescribed medications and medical treatments including administering, storage and side effects. On occasion will be required to pick-up, arrange for renewal, and disposal of outdated or no longer prescribed medications.
- Notify all staff involved, in a timely manner, of changes in client’s medical needs and instruct in the implementation of said changes (i.e., medications, activity limitation, special diet, etc.).
- Maintain charts and files for all residential clients on a constant and regular basis including, the documentation of all appointments, medications, correspondence, and conversations. Keep an up-to-date health record that will be consistently and reliably available to training and support staff, department, and pursuant to WAC 388-101D-0370, the county, community, DDD, and maintain the records’ information privilege and confidential, to be used in the best interest of the client.
- Record observed or reported illnesses, accidents, or changes in functional abilities and dates of actions taken in the health record. Make necessary information available to training and support staff. Maintain all health records information privileged and confidential, to be used in the best interest of the client.
- Participate in any investigation of any accident or incident which jeopardized the health or life of a client and institute measures to prevent similar occurrences in the future as related to health matters.
- Notify client’s next of kin, guardian, or other individual or agency responsible for the client, as rapidly as possible, should a serious change in the client’s condition, transfer of the client to hospital, or death occur.
- Maintain the health records of all dismissed/discharged clients for 4 years. Transfer or allow inspection of records only by authorized, signed consent from the client or legal guardian.
- Coordinate health care between the client, their chosen health professional and their residential program.
- Identify, establish rapport and maintain ongoing professional relationship with medical professionals in the community to provide health care services for our clients. Advocate with health care professionals to acquire best health care options for clients. Facilitate the implementation of all follow-up care requirements.
- Transport clients in personal vehicle or via paratransit to and from medical appointments.
- Perform other duties as assigned.

Essential Requirements:

- Bachelor’s Degree; Preferred
- : 2-3 years, preferred in related or other medical field

- Obtain CPR/ First Aid Certification; within 90 days of hire.
- Attend training required by applicable laws.
- Washington State Driver’s License
- Personal vehicle and proof of insurance of liability coverage $50,000 per accident

Compensation:
- $20 - $25
- Wages are determined by applicable industry experience, education, and certifications/licensure.
Shift: Monday- Friday, Full Time 8am-5pm (1hr. lunch break)
