Position Summary
Thisposition will provide clients with Medical Case management services. The HIVMedical Case Manager will work in close partnership with HIV medical providersin the community. The HIV Medical Case Manager will communicate with providersregarding the client’s case management care plan and the medical treatmentplan. In addition, HIV Medical Case Managers will provide services that manyHIV doctors have limited time to provide: treatment management and medicationadherence, HIV disease and side effects management, nutritional therapy, oralhealth interventions and referral, liver health and hepatitis diseasemanagement (acute and chronic) and HIV risk reduction.
HealthResources Services Administration (HRSA) defines medical case management as:
“Medicalcase management services (including treatment adherence) are a range ofclient-centered services that link clients with health care, psychosocial, andother services.The coordination andfollow-up of medical treatments is a component of medical case management.These services ensure timely and coordinatedaccess to medically appropriate levels of health and support services andcontinuity of care, through ongoing assessment of the client’s and other keyfamily members’ needs and personal support systems.Medical case management includes theprovision of treatment adherence counseling to ensure readiness for, andadherence to, complex HIV/AIDS treatments.
Keyactivities include (1) initial assessment of service needs; (2) development ofa comprehensive, individualized service plan; (3) provision of services requiredto implement the plan; (4) monitoring to assess the efficacy of the plan; and(5) periodic re-evaluation and adaptation of the plan as necessary over thelife of the client.It includesclient-specific advocacy and/or review of utilization of services.This includes all types of case managementincluding face-to-face, phone contact, and any other form ofcommunication.”
Essential Functionsand Responsibilities
Completeregular health assessment on eligible clients
Developand lead RN care plan for eligible clients
Conductcase conferences with Ryan White Care coordinators
ConductCase conference with medical providers, including HIV specialty care
Buildand maintain relationships with medical providers
Completeannual HIV RN training
Advocatefor clients with medical service providers
Providedirect one-on-one medical case management services to eligible clients
Maintainup-to-date knowledge of disease progression and relevant therapies
Provideclient education pertaining to disease progression and relevant therapies
Maintainup-to-date and accurate client records
Advocatewith other case managers and other service providers on behalf of our clients
Provideintensive, short-term intervention to newly diagnosed clients focusing on HIVdisease management, prevention/risk reduction education
Participatein necessary meetings
AssistDirector/Manager with all necessary reports
Participatein agency teams
Travelto other counties to conduct home visits and appointments with clients
Otherduties as defined by Director and Ryan White Principles and Standards
Meetwith clients in person on a drop-in and scheduled basis
RequiredQualifications
ValidRegistered Nursing license
ValidOregon driver’s license, driving record sufficient to be covered by agency autoinsurance policy, ability to transport self to job-related events and educationlocations
Oneyear of experience providing clinical care to patients with chronic disease orinfectious disease

Abilityto pass agency criminal background check
Abilityto work effectively with challenging patients
Abilityto pass AIDS RN Certification within 12 months
Physical Demands
While performing the duties of this job, the employee isrequired
·Repetitive use of hands such asgrasping, twisting/turning of wrists, and finger dexterity necessary to performvarious clerical duties, such as typing and writing.
