Social Services Worker

We are looking for a Social Services Worker.

Social Services Worker Qualifications:

• Must have at least 3 years of Long Term Care experience.
• IA Social Worker License or a Master's Degree in Social Work.
• Knowledge of MDS 3.0 and Point Click Care Knowledge of Electronic Medical Records (EMRs).
• Read, write, and speak English fluently.

Social Services Worker Job Summary:

• Assist with necessary documentation for the resident’s admission to the facility.
• Assist with interviewing applicants and families to determine suitability of residency.
• Interview Resident and family to assess resident psycho-social strengths/ weaknesses.
• Update resident files/charts with the Discharge Plan, Progress Notes, Plan of Care and Assessment.
• Participate in the Interdisciplinary Care Conference Meetings to assess and review Residents’ progress.
• Assist resident and/or family members with issues concerning but not limited to emotional and financial needs.
• Assist in communication with families and notify them of new and apparent needs or any changes of the level of care.
• Conduct duties of the Admission Coordinator when coordinator is not available.
• Invite resident and family to upcoming care conferences.
