Job Summary:

This position requires an experienced Registered Nurse to provide oversight of the medical needs of the adults and children who reside in the Star of Hope Residential Programs and who participate in the Supported Employment and Day Activities Programs. This is a part-time position with variable hours, approximately 5 to 10 hours per week and without benefits. Note: This position oversees the Nurse Delegated Duties for four of our residents so the hours can vary. One week might have five hours, one week might have 8-10. Some weeks could include time logged by taking phone calls from staff, while other time is spent onsite.

Qualifications:

1. Current licensure as a Registered Nurse (RN) in the State of Oregon; minimum one year of experience working with individuals with developmental disabilities preferred.

2. Current valid Oregon Driver’s License with ability to pass a driving record screening.

3. Successfully pass criminal background checks upon hire and a re-check every two years.

Duties:

1. Complete Assessments and Care Plans as required and specifically 30 days prior to the development of Individual Service Plans.

2. Available by telephone for the purpose of response to healthcare emergencies of our supported individuals. All calls will be screened by Program Managers for appropriateness; calls are to be responded to as soon as possible and billed in 15-minute increments.

3. Prepare and submit all required reports, records, and documentation; review on a regular basis all telephone orders, entries into residents’ medical files, and medical administration records.

4. Responsible for nurse delegated tasks throughout the agency, following the Oregon State Board of Nursing rules. All delegation renewals must be performed prior to the expiration date per agency policy and Oregon Administrative Rules, whichever is the higher standard. Maintain all documentation associated with delegations.

5. Attend meetings throughout agency as required.

6. Healthcare liaison when requested between agency and the healthcare community for the purpose of communicating medical needs, working collaboratively with all agency staff and supported individuals.

7. Maintain a working knowledge of the medical and behavioral histories of all supported individuals assigned while maintaining a nurse/patient relationship with them.

8. Attend appropriate training opportunities as required and as agreed upon for maintaining nursing license.

9. Other duties as assigned and additional duties may be added as mutually agreed upon.
