Job Description:
Are you an enthusiastic, patient-centered nurse residing in the state of Missouri? Join our dedicated team of Care Managers specializing in providing comprehensive care for aging adults. Our team delivers chronic care management services telephonically to patients across Missouri, ensuring high-quality care and support for seniors in skilled nursing, assisted living, and other long-term care facilities.
Key Responsibilities:

• Provide telephonic chronic care management to aging adults across Missouri.
• Develop and implement personalized care plans tailored to each patient's needs.
• Monitor and assess patient progress, adjusting care plans as necessary to ensure optimal outcomes.
• Collaborate with other healthcare providers and facilities to coordinate patient care and ensure a cohesive approach.
• Educate patients and their families on managing chronic conditions and the importance of adhering to care plans.
• Document all patient interactions, assessments, and care plans accurately in the electronic health record system.
• Ensure compliance with all healthcare regulations and standards of practice.

Requirements:

• Must be a licensed nurse (LPN) in the state of Missouri.
• Ability to travel to Columbia, St. Louis, or Kansas City as needed.
• Strong communication and interpersonal skills.
• Proficient in using computers and various software applications.
• Self-motivated and able to work independently in a remote setting.
