Job Summary
Manages the daily operation of the Case Management Department staff to provide effective and efficient discharge planning services to patients in accordance with policies, procedures, regulatory requirements consistent with the mission, vision and values of the Jefferson Health Enterprise.
Care Management Competencies
Care Coordination
Knowledge of the medical, social, economic, and other services provided by other departments or facilities, and the ability to integrate these multidisciplinary treatments for optimal patient care.
Healthcare Regulatory Environment
Knowledge and understanding of legislation and regulatory bodies affecting healthcare practices.
Interpersonal Relationships
Knowledge of the techniques and the ability to work with a variety of individuals and groups in a constructive and collaborative manner.
Managing Multiple Priorities
Ability to manage multiple concurrent objectives, projects, groups, or activities, making effective judgments as to prioritizing and time allocation.
Qualifications:
Required
RN BSN required
Minimum of 3 years of Case Management or Homecare discharge planning experience required
2 years of previous Manager or Leader experience required
Position may require floating to all 3 Jefferson East Hospitals
Position may require Manager to manage a caseload of patients
KPI management
Weekend on call rotation
Holiday on call rotation
Preferred
CCM certification preferred, will be required within 2 years of hire.
EPIC EMAR experience preferred
