Make a Lasting Impact: Nurse Practitioner (NP)

Are you a passionate NP seeking to provide compassionate care to a deserving senior population?

Our facility is seeking a dedicated and experienced Nurse Practitioner (NP) to join our team and deliver high-quality primary care to our residents. In this fulfilling role, you'll play a vital role in ensuring the health and well-being of our residents, working collaboratively with our interdisciplinary team to create personalized care plans.

Responsibilities:

  • Conduct comprehensive physical examinations, diagnose and treat acute and chronic illnesses in a geriatric population.
  • Develop and implement individualized care plans that address residents' medical, physical, and psychological needs.
  • Prescribe medications, order and interpret diagnostic tests, and perform procedures within your scope of practice.
  • Collaborate with physicians, nurses, social workers, and therapists to ensure coordinated care.
  • Educate residents and families about health conditions and treatment options.
  • Advocate for residents' well-being and ensure they receive appropriate care.
  • Document patient care accurately and thoroughly in the electronic health record (EHR).

Qualifications:

  • Active certification as a Nurse Practitioner (NP) with a focus on geriatrics.
  • Minimum 2 years of clinical experience in a geriatric setting (nursing home, long-term care, or geriatric clinic).
  • Strong clinical skills in geriatric assessment, diagnosis, and treatment.
  • Excellent communication, collaboration, and interpersonal skills.
  • Compassionate and patient-centered approach to care.
  • Proficient in EHR documentation.
