Community Health and Wellness of Greater Torrington, Inc. is Hiring a Physician Internal Medicine Near Canaan, CT

Reporting to the Medical Director, specializes in adult medicine, and is trained to solve diagnostic problems, manage long-term illnesses, and help patients with multiple, complex chronic conditions. Is responsible for ensuring coordinated and integrated care along service lines. Will examine and treat patients with a wide range of conditions and refer those that need a higher level of care to a specialist or appropriate facility. Typical job duties include taking patient histories, conducting physical examinations to make diagnoses, prescribing treatments and counseling patients about lifestyle changes that can lead to health improvements.
Essential Functions & Responsibilities:
Experience in primary care setting preferred. Ability to work as part of a multidisciplinary team. Interest in serving low income-multicultural populations. Demonstrates the knowledge and skills necessary to provide care appropriate to the age of the patients served. Ability to identify and categorize each patient’s age specific grouping of needs, such as those for adolescent and geriatric patients. The ability to establish and maintain collegial relationships with coworkers, and to function in a calm manner during stressful and emergency situations. Demonstrates flexibility and resilience in response to changing clinical or administrative environments. Must have valid driver’s license from State of residence. Must be able to transport oneself within the service area.
Major Duties & Responsibilities:
  • Experience in primary care ambulatory setting preferred.
  • Ability to perform patient medical histories.
  • Conduct thorough physical examinations.
  • Ordering appropriate diagnostic imaging and bloodwork
  • Ability to make appropriate diagnosis by analyzing data.
  • Prescribing appropriate medications and treatments.
  • Developing patient treatment plans with patient and caregiver involvement.
  • Ability to modify treatment plans based on patient progress.
  • Updating patient records to accurately reflect current status.
  • Has the ability to establish and maintain collegial relationships with coworkers, and to function in a calm manner during stressful and emergency situations.
  • Demonstrates flexibility and resilience in response to changing clinical or administrative environments. 
  • Provide quality care to patients according to evidence based medical protocols and guidelines while delivering quality health services to patients of diverse cultural, ethnic, and economic backgrounds.
  • Initiate an appropriate assessment of patient's health status, ranging from focused to comprehensive, depending on patient need.
  • Provide and manage comprehensive medical care, including diagnosis and nonsurgical treatment of diseases, common health problems, such as infections, influenza and pneumonia, as well as serious, chronic, and complex illnesses for adult and geriatric patients.
  • Monitor patients' conditions and progress and schedule appropriate follow up care.
  • Explain procedures and discuss test results or prescribed treatments with patients.
  • Advise patients concerning diet, activity, hygiene, and disease prevention.
  • Prepares reports and correspondence for referrals.
  • Prescribe medication in accordance with CT statue and professional practice guidelines.
  • Participate in formulating, interpreting, implementing, and evaluating policies, procedures and workflows.
  • Assist with quality improvement activities as required and participates in Quality Assurance and Utilization Reviews.
  • Participate in Accountable Care Organization activities as required.
  • Perform diagnostic tests and procedures and therapeutic procedures, within the scope of practice, including but not limited to intramuscular and subcutaneous injections; electro-cardiogram; application of dressings and bandages; administration of medications; cardiopulmonary resuscitation; auditory screening; visual screening; and aseptic techniques.
  • Have a working knowledge of ICD10, CPT and HCPCS coding and managed care, and work collaboratively with the billing for training as necessary.
  • Participate in the on-call rotation schedule as required.
  • Comply with the regulations and policies required of a Federally Qualified Health Center.
  • Complete administrative assignments as requested by Medical Director and/or Center’s administration.
  • Perform other duties as assigned to support the Center’s Mission, Vision, and Values.
  • Train residents, medical students or newly hired providers when necessary.
Additional General Requirements: Professional positive attitude, understanding of customer service principles, trustworthiness and excellent interpersonal skills.
Job Qualifications/Requirements:
M.D./D.O. degree from an accredited school.
Documented completion of residency from an accredited institution in an appropriate specialty. Board certification or Board eligibility desirable.
 Current licensure in the State of Connecticut.
Valid Federal and State Controlled Substance Licenses.
Responds constructively to pressure situations and time limits.
Able to accept constructive criticism and instruction while maintaining a positive attitude towards patients, managers, programs and job responsibilities.
Acts as a role model – willing to accept or volunteer on projects as opportunity arises.
