SYNERGY HomeCare is hiring for a Lead Caregiver for a couple in the South Bay. The Lead Caregiver will work 4 shifts per week 8am to 8pm, as part of a 24 / 7 care team for our clients.

The Lead Caregiver will be the main point of contact for the SYNERGY Care Support Team (office staff) and the Client. The Lead Caregiver will be responsible for ensuring the accurate implemenation of the care plan across all caregivers and shifts.

Requirements for Lead Caregiver serving our Couple Care Client :

• Experience working with Clients with Dementia, Hospice, and recovering from injury or illness
• Ability to work collaboratively with other care professionals
• Must be comfortable providing personal care for both male and female
• Ability to cook, and follow nutritional guidelines
• Have HCA registration, or eligible for HCA (Ability to pass a background check & TB test)
• Have reliable transportation, and are committed to excellent care for you clients.

As the Lead Caregiver for a couple, you will :

• Take execellent notes on meals, medication reminders, fluids, exercise, etc.
• Provide support to caregivers, and coordinate schedules to ensure consistent care.
• Provide attention to clients’ non-medical needs, including companionship and social engagement.
• Assist with light housekeeping, meal preparation, and medication reminders.
• Establish communication and a professional relationship with clients, family members, and co-workers.
• Work with SYNERGY Care Manager to ensure consistency and quality of care across all caregivers and client shifts
• Provide reliable care by being punctual and consistently covering shifts.
