Healthcare Support is Hiring a Field RN Case Manager Near Bristol, CT
- Assist the provider/team with various care coordination activities.
- Assist Providers in assuring complete and accurate documentation and coding, medication reconciliation, nursing rounds, assessment, and patient documentation.
- Assist in managing members through an acute medical situation or an exacerbation of a chronic condition, including testing, diagnostics, interventions.
- This position requires travel between three (3) facilities.
- Monday – Friday, 8AM-5:00PM
- Active and unrestricted license in Connecticut.
- Minimum 1 year working in a hospital or Long-Term Care Facility as an RN.
- Associate’s degree or higher in Nursing (RN).
- Certified in Basic Life Support.
- Ability to complete physical requirements, including lifting a 30-pound bag and ability to perform physical assessment.
- Access to reliable transportation.
- Full COVID-19 vaccination is an essential requirement of this role.
