Overview:

Works within general direction of the Hospice Manager or Designee in accordance with agency policies; prepares psychosocial evaluations; provides medical social casework to patients and families having difficulty in social functioning, primarily as it effects their health; refers and aids in the referral process to social agencies; and generally services as a medical social worker in the agency’s total program.

Responsibilities:

• Assess patient and family dynamics, considering family relationships, cultural attitudes, and financial impact of illness.
• Offer social casework to aid understanding of medical recommendations and restore patient's social and health adjustment.
• Document evaluations, treatment plans, and communication with patients and team members.
• Address referrals for evaluation or casework, following treatment plan.
• Create a treatment plan based on evaluation findings.
• Supervise and guide social work students and interns as needed.

Job Type: Contract

Pay: $60.00 - $61.00 per hour

Expected hours: 30 per week

• license to work in Michigan as Social Worker (Preferred)
