Carriage House Nursing and Rehab working towards our mission to provide quality long-term care and related health services is looking for caring Licensed Practical Nurses for our fantastic residents and patients.

Responsibilities will include but are not limited to:

• Administers medications to patients as prescribed by the physician and advise patients and families regarding changes to medications, treatment plans, and patient condition.
• Oversees the CNA team including giving direction & assignments, offering training, and contributing to recognition and evaluations.
• Provides preventative and supportive care directed to the physiologic and psychosocial well-being of patients.
• Assesses and anticipates the needs of the patient, and coordinates the necessary interventions as defined by policy, procedure, and the written or verbal orders of the provider as applicable to assigned area(s).
• Receive/give the nursing report upon reporting on/off duty and main other records as directed.

Ideal candidates will possess the ability to effectively communicate with patients and other staff. Prior experience as skilled nursing/rehabilitation preferred but not required.

Educational requirements:

• Graduate of accredited Nursing School
• Licenses/Certification- Valid Michigan Nursing License (LPN)
• BLS CPR certification required.
