TRI-COUNTY CARE LLC is Hiring a Care Manager Near Argyle, NY

Job Overview:
The role of the Care Manager is to work collaboratively with individuals with intellectual/developmental disabilities, their families, OPWDD, and relevant providers to ensure that all individuals supported are able to live safe, happy, and fulfilling lives. This includes working to identify goals, building plans to reach those goals, and assistance with applying to a variety of supports and services to put the individual on the right path to meet their person-centered outcomes. Tri-County Care supports members of all ages – this may include advocating for individuals with their schools, helping them to find medical providers, assisting them with applications to Social Security, SNAP benefits, Waiver programs, etc.

Location: This position is remote but does require periodic in-person visits. Candidates will need to be able to travel to do these visits via car or public transportation. Caseload to be located in or near listed county -but is not limited to the county listed.
This position requires applicants outside of New York City to have a valid driver's license and reliable transportation. Travel reimbursement is provided.
This position requires a quiet distraction-free environment for working, or the ability to work from one of our regional offices.
This job description is not all inclusive and the employee may be asked to assume additional responsibilities as the need arises.
Specific Knowledge, Skills, and Abilities:

  • Excellent communication skills (both written and verbal)
  • Conflict-management and problem solving skills.
  • Computer software skills, particularly skills with Microsoft programs (Word, Excel, Outlook)
  • Ability to multi-task, organize, schedule, and utilize time well
  • Capability to analyze situations accurately, prioritize, and take effective action.

Required Education, Experience, and Licenses:

  • A bachelor’s degree with two years of relevant experience, OR
  • A License as a Registered Nurse with two years or relevant experience, which can include any employment experience and is not limited to case management/service coordination duties, OR
  • A master’s degree with one year of relevant experience
  • MSC Service Coordinators prior to July 1, 2018 are “grandfathered” to facilitate continuity of care
