Community Partners is looking for a friendly, motivated and outgoing individual to join our team supporting individuals with mental health / behavioral health needs.

We are looking for a Team Leader for Adult Services. This role will have primary responsibility for clinical oversight and direction of assigned cases and provides oversight / supervision to a multidisciplinary team of Masters and Bachelors level providers working together to meet the health needs of assigned cases.

Specific Responsibilities will include, but not be limited to :

  • Performs clinical oversight and direction of assigned cases.
  • Oversees the success of a multidisciplinary team of masters and bachelors level providers
  • Providers training, oversight, mentoring and evaluation of multidisciplinary team.
  • Orients new staff, under the direction of the Department Manager.
  • Provides oversight, direction and training to the treatment team regarding assessments, treatment planning, treatment delivery, and discharge planning of all assigned cases in collaboration with the team psychiatrist.
  • Monitors the utilization of services assuring that assessments, treatment plans and progress notes are aligned to produce desired outcomes.
  • Conducts staff meetings; facilitates cooperation among staff to enhance team development and collaborative clinical work.
  • Completes employee performance evaluations.
  • Maintains program operations consistent with applicable state and federal laws, rules and regulations in addition to Community Partners Quality Improvement Program.
  • Works collaboratively with members of the QI Department to resolve complaints.
  • Maintains a small caseload as needed.
  • Demonstrates a caring, professional attitude and ethical behavior toward clients, client families, staff and collateral contacts within the agency and community.
  • Understands the relationship between service provision and method of payment.
  • Attends meetings, trainings and events as assigned.
  • Performs tasks, projects and other duties as assigned.

Qualifications :

  • Masters Degree required in Social Work, Psychology, Marriage and Family Therapy or related field.
  • Three (3) years of relate clinical experience required.
  • Two (2) years of supervision experience preferred.
  • Licensed or license eligible by the NH Board of Examiners of Psychology and Mental Health Practices preferred.
  • Ability to plan and perform a wide variety of duties related to program administration.
  • Ability to exercise independent judgement and use discretion in the performance of these duties while operating within the limits of standard practice.

All candidates must pass a background check which includes, but is not limited to, criminal and driving records.
