Position Overview: TXRN is seeking a dedicated and compassionate Hospice Liaison to join our team in Mansfield, TX. The Hospice Liaison will serve as a crucial link between our organization and healthcare professionals, patients, and families in the community. This position plays a vital role in facilitating smooth transitions to hospice care and ensuring that individuals receive the support and resources they need during challenging times.

Responsibilities:

• Develop and maintain strong relationships with healthcare providers, including physicians, nurses, social workers, and case managers, to promote TXRN’s hospice services.
• Educate healthcare professionals and community organizations about TXRN’s programs, services, and eligibility criteria for hospice care.
• Collaborate with clinical teams to assess patient needs, coordinate admissions, and facilitate timely transitions to hospice services.
• Conduct comprehensive assessments of patients’ physical, emotional, and spiritual needs to ensure personalized care plans are developed and implemented.
• Serve as a resource and advocate for patients and families, providing support, guidance, and assistance throughout the hospice journey.
• Participate in community outreach activities, including educational events, health fairs, and support groups, to raise awareness of hospice care and TXRN’s services.
• Maintain accurate documentation of interactions, referrals, and outcomes in accordance with regulatory requirements and organizational policies.

Qualifications:

• Bachelor’s degree in nursing, social work, healthcare administration, or related field preferred.
• Minimum of 2 years of experience in healthcare sales, marketing, or liaison role, preferably in hospice or palliative care.
• Strong interpersonal skills with the ability to build rapport and communicate effectively with diverse stakeholders.
• Knowledge of hospice regulations, Medicare/Medicaid guidelines, and healthcare referral processes.
• Ability to work independently, prioritize tasks, and manage multiple responsibilities in a fast-paced environment.
