Under the general supervision of the Director of Pharmacy, the staff pharmacist is responsible for the performance of professional functions generally associated with the practice of pharmacy in the institutional setting as authorized by the Vermont Board of Pharmacy Rules and Regulations. Reviews and interprets physician orders to evaluate drug incompatibilities, allergies, appropriateness of therapy, including drug selection, dosage, and frequency of administration. Confers with the physicians and allied health personnel as necessary to resolve all questions and irregularities regarding physician orders. Demonstrates knowledge of high alert/high risk medications and steps to ensure safe use. Demonstrates appropriate clinical knowledge with use of standard protocols to improve medication safety.
