**Job Overview:** We are seeking a dedicated Case Manager to join our team. The Case Manager will be responsible for providing support and guidance to individuals in need, helping them navigate various social services and resources. **Responsibilities:** - Conduct assessments to identify clients' needs and develop individualized care plans - Provide crisis intervention and emotional support to clients in difficult situations - Utilize motivational interviewing techniques to empower clients to set and achieve goals - Offer mentoring and guidance on addiction counseling, childcare, Medicare, Section 8 housing, workforce development, and other relevant areas - Collaborate with healthcare providers, social workers, and community organizations to coordinate services for clients - Monitor client progress and adjust care plans as needed - Maintain accurate and confidential client records **Requirements:** - Bachelor's degree in Social Work or related field; Master's degree preferred - Proven experience in case management, social work, or related field - Knowledge of behavioral health issues and resources - Strong communication and interpersonal skills - Ability to work effectively in a fast-paced environment - Familiarity with social services programs and community resources This position offers the opportunity to make a meaningful impact in the lives of individuals in need.
