Position: Hospice Clinical Manager
Department: Hospice
Reports to: Director of Hospice

Location: Fayetteville, NC
Position Type: Full-time

Job Scope:
The Hospice Clinical Manager oversees the coordination and management of patient care within the hospice department, ensuring that services are delivered in accordance with the highest standards of quality and compliance. In addition, as a vital member of the management team, the Hospice Clinical Manager works with the Hospice Director to oversee the clinical appropriateness of all new and current clients and to assist in effectively managing the day-to-day operations of the hospice clinical staff. This role is critical in fulfilling HealthKeeperz’s mission to care for all people for the glory of God as we deliver compassionate and excellent care.

Expectations:

  • Patient Care Coordination: Manage the intake, evaluation, and assignment of clinical staff to new cases. Conduct comprehensive case reviews and home visits to assess care quality.
  • Clinical Supervision: Supervise clinical personnel, ensuring proper care management and adherence to care plans. Provide ongoing education and support to clinical staff.
  • Quality Assurance and Compliance: Ensure compliance with professional standards and participate in quality assessment and performance improvement activities.
  • Interdisciplinary Team Leadership: Lead regular case conference meetings to discuss patient care, involve interdisciplinary group discussions, and ensure effective communication among team members.
  • Operational Management: Oversee administrative and operational aspects of the hospice program, including staff scheduling, budgeting, and resource allocation.

Essential Responsibilities:

  • Case Management: Receive and review case referrals, assigning appropriate personnel and scheduling initial evaluations within required timelines.
  • Care Oversight: Monitor patient care processes, review medical documentation, and adjust care plans as necessary to meet patient needs.
  • Staff Development: Conduct training sessions, performance evaluations, and provide mentorship to foster professional growth and high performance.
  • Compliance Monitoring: Ensure all practices and procedures comply with healthcare regulations and ethical standards.
  • Quality Improvement: Participate in and lead quality improvement initiatives and teams, aiming to enhance care delivery and patient satisfaction.
  • Emergency Response: Available for urgent consultations with staff during operating hours to address immediate patient care needs.
  • Documentation and Reporting: Ensure accurate and timely documentation and reporting per hospice policy and regulatory requirements.

Qualifications:

  • Education: Registered Nurse with a BSN preferred; minimum requirement of an ADN.
  • Licensure: Current RN licensure to practice in North Carolina.
  • Experience: At least two years of management or supervisory experience in hospice, home health, or community health settings within the last five years.
  • Skills: Strong leadership, communication, and organizational skills; proficient in nursing judgment and care planning; understanding of hospice philosophy and principles of pain/symptom management.

Certifications:

  • Current CPR certification.

Physical Demands:

  • Frequent travel to patient homes, ability to handle occasional lifting, and extensive periods of standing or sitting as required.
