Independence Associates, Inc. is Hiring a RN/LPN Contractor (PCA Program) Near East Bridgewater, MA

Position: Evaluator for the (PCA) Personal Care Attendant Program

Purpose: To evaluate the physical and cognitive functional status of applicants in their home and make recommendations to maximize their safety and independence.

Supervisor: PCA Program Coordinator

Reimbursement: $75.00 for PCA evaluations

$37.50 for no show appointments

$0.50 per mile

Responsibilities:

  • To assess and identify the consumer’s needs and functional capabilities including the ability to manage personal care services.
  • To evaluate the consumer’s needs in their home environment to maximize safety and independence.
  • To make cost effective recommendations on equipment, services, physical therapy, occupational therapy and skills training.
  • To work in tandem and exchange information with the Occupational Therapist during and
  • To complete and submit all necessary documentation as provided by Independence Associates, Inc. so that a successful application can be made to the Division of Medical Assistance.
  • To complete and submit all necessary documentation on a timely basis (i.e., evaluations, billing slips, etc.)
  • To maintain current on state and federal regulations of the MassHealth Personal Care Attendant Program and any internal policies and procedures of Independence Associates, Inc.
  • To ensure that persons with disabilities have maximum control over their PCA program.
  • To re-evaluate consumers’ needs in their home on an annual basis or as needed.(in case of two year PA’s, evaluation will be done every other year.)
  • To maintain current licensure as a Registered Nurse in Massachusetts.
  • Increase request evaluations; occasionally in person typically by phone.
  • Attend initial orientation.

Qualifications

Must be a currently Registered Nurse in Massachusetts; have experience with persons with disabilities in the community at large; have excellent communication and paperwork skills; be well organized, self motivated and have transportation.
