Job Title: Mobile Clinician
Program: Single Homeless Assessment Center (SHAC)
Salary Range: $60,000 - $70,000 (Salary Based on experience and education)
Hours: 35 hours per week (8 am- 4 pm, no after-hours or on call required)
Position: In-Person (Mobile as needed)

SUMMARY Of DUTIESThe Mobile Clinician is a member of an interdisciplinary, interagency assessment team and is located on-site at the Single Homeless Assessment Center (SHAC). The candidate is part of the clinical professional staff of the licensed clinic. The primary responsibility of the mobile clinician is to conduct assessments of individuals who are currently homeless and who are requesting housing assistance. Assessments will take place at the various homeless shelters, Department of Social Services offices or in the community. The focus will be to determine the individual’s needs for specific supports necessary for independent living and to conduct comprehensive assessments (e.g. mental health, substance use, educational/vocational) to determine service needs. The Clinician will support recovery by providing services that reflect our deeply held values that include the recognition of individual rights of self-determination, choice, shared decision-making and collaboration. When possible the candidate will utilize evidence-based practices; always embracing a trauma-informed and person-centered approach.
SPECIFIC DUTIES And RESPONSIBILITIES
  • Conducting thorough mental health and substance use assessments for individuals residing in participating homeless shelters.
  • Active participate in daily case conference
  • Collaborating closely with staff from homeless shelter, the Department of Social Services (DSS), and on-site medical providers.
  • Ensuring assessments are completed promptly and making referrals as required.
  • Playing an integral role within a team to fulfill job responsibilities.
  • Providing crisis intervention, if necessary, for individuals and staff on-site at the shelter
  • Maintaining comprehensive documentation of assessments in adherence to established methods and procedures.
  • Engaging in program development and training programs
  • Strictly adhering to established clinic policies and procedures
  • Maintain appropriate professional standards.
REQUIRED KNOWLEDGE And SKILLS
  • Familiarity with and ability to use an Electronic Health Record system.
  • The SHAC Clinician must be reliable and responsible, demonstrating the ability to work both independently and as part of a team.
  • Applicable city, state, and federal guidelines and laws affecting mental health treatment, services, and reporting procedures.
  • Knowledge of the common challenges associated with mental health conditions.
  • Principles of psychological growth and development.
  • Community mental health resources and services and other health, financial, social, recreational cultural, housing, and legal advocacy services.
  • Understanding the impact of culture on mental health practices.
Qualifications
  • The candidate must hold one of the following New York State licenses: LMSW, LMHC, Psychologist, or LCSW.
  • Must be reliable and able to work independently and understand the importance of maintaining confidentiality.
  • Have excellent interpersonal and communication skills.
  • Work independently, and complete tasks and all required paperwork in a timely and complete manner.
  • Must have advanced computer skills in Microsoft Office - Excel, Word, PowerPoint, and database management.
  • Have and maintain a valid Driver’s License with a driving record that is satisfactory to our insurance carrier and a Defensive Driving certification.
  • The Clinician must be cleared NYS Justice Center through fingerprinting for a Criminal History Records search.
  • The Clinician must be cleared by the Justice Center for the Protection of People with Special Needs Staff Exclusion List (SEL) and the New York State Medicaid Exclusion List.
  • The Clinician must be cleared by the NYS Office of Children and Family Services (OCFS) for instances of child abuse and/or neglect.
  • The Clinician must be legally eligible to work in the United States.
