As a Certified Nursing Assistant (CNA) you will perform personal care activities related to the activities of daily living, under direction of a Case Manager RN.

QUALIFICATIONS for Certified Nursing Assistant (CNA):

• Must be willing to commute to patients within a broad territory in Minnesota. Gas reimbursement provided.
• Must have reliable vehicle.
• Caregiving experience required (Home Care, Assisted Living/Memory Care, Skilled Nursing/Nursing Home, Hospital or Hospice)
• Hospice experience preferred
• Excellent customer service skills
• Current CPR Certified
• Valid driver's license and auto insurance
• Valid CNA License

RESPONSIBILITIES of Certified Nursing Assistant (CNA)(including but not limited to):

• Personal cares (bathing, dressing, grooming, toileting, skin care, etc)
• Assistance with household cleaning
• Review patient care plan
• Lifting and transferring
• Assistance with range of motion & positioning, exercise & ambulation
• Assistance with dining & nutrition
• Electronic charting
• Exhibit and communicate the values of Favoured Hospice
• Ability to work as a member of a team
• Provide excellent communication to staff and family/provide updates on patient condition

In addition:

• Must be accountable
• Must be able to lift 50 lbs
• Good understanding of body mechanics
• Strong communication skills-written, verbal, listening with attention
• Good problem-solving skills
• Good time management
• Able to function in an electronic medical record
