Favoured hospice is Hiring a Physical Therapist - Hospice Near Dayton, MN

Our hospice team is looking for a Physical Therapist to join our team. The role is primarily focused on hospice patients, wherever they consider home. This could be a private home or nursing facility.

The team offers mileage reimbursement and scheduling based on your availability.

This position is ideal for someone who is looking for a part time flexible position, and comfortable with increasing hours as we grow.

Compensation per visit.

CURRENT TERRITORY: Maple Grove, Minnetonka, Roseville, Anoka/Ramsey. However, must be willing to travel throughout state of MN as we expand.

JOB SUMMARY:

The Physical Therapist provides hospice services including, but not limited to:

  • Perform initial evaluations and assessments of patients' conditions
  • Develop and revise a Plan of Care geared towards patient comfort and pain relief
  • Evaluate outcomes of care with appropriate discharge planning
  • Effective interpersonal relations and communication skills to advise and consult with family and other agency personnel
  • Utilize trigger point therapy to alleviate pain and promote healing
  • Educate patients on proper body mechanics and exercises for safe mobility and fall preventions
  • Educate family members on safe positioning techniques/transfer devices
  • Complete appropriate documentation in timely manner
  • Attend staff interdisciplinary team meetings once every 2 weeks

JOB CONDITIONS:

Position requires deadlines and timelines required by governing and regulatory organizations.

Sitting, standing, stooping and walking, as well as telephone communication are required. Face to face visits may entail moderate physical exertion and car travel.

Hearing, eyesight and physical dexterity must be sufficient to perform a physical assessment of the patient's condition and to perform and demonstrate patient care.

Adhering to regulations, standards and conditions of participation required by Medicare, accrediting body, and federal and state statutes governing the provision of services.

QUALIFICATIONS

Must have current Minnesota License as a Physical Therapist

Current Driver’s License

Has experience with end of life care.

Must have the ability to demonstrate self-confidence and positive attitude toward self and others and maintain commitment and enthusiasm to goal achievement.

The ability to communicate well, both verbally and in writing.

Job Type: Part-time

Expected hours: 6 – 12 per week
