The Home Visiting Program Manager is accountable directly to the Director of Maternal and Child Services. Provides oversight to the Parents As Teachers (PAT) and Healthy Families America (HFA) home visiting program ensuring Family Case Managers activities meet requirements of the grants and PAT/HFA home visiting model fidelity. Coordinates all activities to ensure the program retains families, improves parent skills and confidence, and prepares children for school success. This is a full-time position.
Highlighted Duties and Responsibilities:
Provides leadership and management of the home visiting program ensuring full implementation of the Parents As Teachers and Healthy Families America models and maintains accreditation.
Per the terms of the grant, administers client satisfaction surveys to participants of HFA and PAT.
Develops partnerships and manages referrals to behavioral health and other services.
Maintains accreditation status with national model staff.
Monitors Child and Family Services Administration referrals and ensures that they are recorded as Unusual Incident Reports. Ensures adequate follow up on all referrals and reports
Supervises staff implementation of the Parents As Teachers and Healthy Families America home visiting models
Ensures that all home visiting staff are properly trained to enter data in required data systems (ex. eCW ,DCRS local and HV Recruitment Portal) within four (4) working days of available information and data.
Provides support to Family Case Managers to ensure that the required numbers of home visits are conducted on a monthly basis and all documentation is completed accurately and timely.
Prepares and submits reports to funders according to contract deadlines. Ensures reports and documentation address targets and measures outcomes for activities. Ensures that home visiting staff are aware of targets and outcomes
Conducts weekly reflective supervision with each Family Case Manager and the Family Assessment and Data Specialist
Provides support to Family Case Managers to ensure that the required numbers of home visits are conducted on a monthly basis and all documentation is completed accurately and timely.
Ensures staffs are completing required assessments ( Parent Survey, ASQ-3, ASQ:SE-2, PH-Q9, Adult Adolescent Parenting Inventory, Abusive Behavior Inventory) within established times frames.
Partners with the Talent Management team regarding personnel matters including but not limited to performance management, terminations, leaves of absence, recruiting, training, and onboarding.
Minimum Qualifications:
B.A. in Human Services related field (social work, early childhood education, family studies, and/or psychology) or equivalent professional experience required.
If applicant has a BSW/MSW degree, LSWA/LGSW or appropriate social work license obtained within 90 days of hire
Experience with providing public health case management and education (i.e., reproductive health, smoking cessation, child development, and parenting) to at risk populations preferably in the clients home setting strongly preferred
At least 2 years of supervisory experience required.
At least 2 years of experience in public health and/or human services field required.
Knowledge of community resources or the ability to become knowledgeable required.
Ability to conduct home visits. Valid driver’s license and vehicle required, as well as proof of auto insurance required.

In relation to remote work vs on-site expectations, this position is classifies as the following: Hybrid: Remote work and on-site work requirements are generally split roughly 50/50.
