We are seeking a highly motivated and experienced Licensed Clinical Social Worker (LCSW) to provide telehealth counseling services to older adults in residing throughout New York. The LCSW will be responsible for conducting assessments, developing treatment plans, and providing individual and/or family therapy to clients in their homes.

Key Responsibilities:

• Conduct comprehensive assessments of clients' social, emotional, and psychological needs
• Develop individualized treatment plans and set goals in collaboration with clients and their families
• Provide individual and/or family therapy to clients.
• Monitor clients' progress and make adjustments to treatment plans as needed
• Communicate with other healthcare providers to ensure continuity of care

Qualifications:

• Must hold a valid LCSW license in the state of New York
• Must have experience working with older adults
• Strong clinical and diagnostic skills
• Excellent communication and interpersonal skills
• Must be able to work independently and as part of a team
