The Wellness Coordinator is a PT position (LPN or RN license required) working with the geriatric population in a personal care community. This position includes providing hands-on care to residents while working with co-workers to develop a complete assessment of the resident and their needs and then formalizing that support plan in the required documentation to provide continuity of care. It is a position with a mix of administrative as well as hands-on duties so that our residents receive the highest quality of personalized care, which is clearly reflected in the appropriate documents.

The candidate must be detail oriented, good at multitasking and problem-solving. The Wellness Coordinator must work well with others and independently. Communicates regularly with staff, residents, family members, and other HCP.

The ideal candidate must possess a current PA License, and previous experience in a supervisory role, and experience working with seniors. Candidates must have strong organizational and multi-tasking skills and proficiency with computers. 

This position is responsible for admissions, re-admissions, discharges, scheduling appointments along with transportation, working with prescribers to clarify orders, working with outside HCP to meet residents’ needs.

Once training is completed, regular weekly schedule would be 3 shifts between Monday-Friday from 8-5pm. Occasionally may need to stay past 5pm to complete duties. Weekends may be required occasionally.

Responsibilities

• Documenting resident care needs by completing and keeping Assessments and Support Plans up to date according to regulations.
• Audit medications and resident files, and resident suite to ensure compliance with regulations and policy.
• Review orders, complete assessments, admissions and re-admissions.
• Mentor through training and education (primarily for Nurse/PCA's/Med Tech)
• Ensure consistency in resident's care by providing hands-on care and assessing changes needed to meet each resident's needs.
• The ability to administer medications and work the floor.
• Responsible to be back-up on call and support staff needing to cover Nurse/Med Tech shifts.
• Work well with others.

Qualifications
