In 2001, the doors opened to The Grand Residence at Upper St. Clair, a senior living community offering 70 guest suites and a host of unique amenities. The Grand Residence is a licensed personal care home. The spacious suites range in size from 400 square foot efficiencies to 750 square foot one bedroom suites. All suites feature a kitchenette with refrigerator, microwave, cabinets and sink; private bathroom; and individual climate control. The Grand Residence, which brings an unprecedented level of luxury and service to the South Hills of Pittsburgh, was based upon a hospitality model and designed with all the luxurious appointments of a grand hotel.

Job Description

This is the perfect position for an LPN or RN, who likes a mix of hands-on nursing and administrative duties and enjoys working with the geriatric population in a personal care community.

The Wellness Director is a FT position; LPN or RN license required. This position includes providing hands-on care to residents while working with co-workers to develop a complete assessment of the residents and their needs and then formalizing that support plan in the required documentation to provide continuity of care. It is a position with a mix of administrative as well as hands-on duties so that our residents receive the highest quality of personalized care, which is clearly reflected in the appropriate documents.

This position is on call for emergencies and to provide support in case of a nurse/med tech call off. The candidate must have the ability to work the floor, audit shifts (on a rotating basis), all days (rotating basis). Be responsible to receive call offs from Nurses and Med Tech staff and cover shifts. The Wellness Director will assist with recruiting, interviewing, on-boarding Wellness staff, and ongoing education. They will manage the training for Nurses, PCA's, and Med Techs. Ensure staff training is completed, documented, and stored timely according to policy and state regulations. The Wellness Director must work well with others and independently. Communicates regularly with staff, residents, family members, and other HCP.

The ideal candidate must possess a current PA License, and previous experience in a supervisory/management role, and experience working with seniors. Candidates must have strong organizational and multi-tasking skills and proficiency with computers. This is a management position.

Once training is completed, regular weekly schedule would be 4 shifts between Monday-Friday. Would be a mix of 8, 10 and 12 hour shifts equaling 40-45 hours a week. Weekends may be required occasionally.

Responsibilities

  • Documenting resident care needs by completing and keeping Assessments and Support Plans up to date according to regulations.
  • Assist in recruiting, interviewing, on boarding wellness staff and ongoing staff education.
  • Audit medications and resident files, and resident suite to ensure compliance with regulations and policy.
  • Review orders, complete assessments, admissions and re-admissions.
  • Mentor through training and education (primarily for Nurse/PCA's/Med Tech)
  • Oversee all required staff training and education is completed according to policy and state regulations.
  • Ensure consistency in resident's care by providing hands on care and implementing protocols to meet each resident's needs.
  • Schedule staff training.
  • The ability to administer medications, work the floor, audit all shifts, all days (periodically as needed).
  • Responsible to help be on call and support staff needing to cover Nurse/Med Tech shifts.

  • Complete Nurse/Med Tech schedule
  • Report directly to Guest Service Director and Executive Director

Qualifications

  • Must be a LPN or RN (with PA license in good standing)

  • Excellent telephone skills
  • Possess strong computer skills, including PointClickCare
  • CPR/1st Aid Certified (or completed within 30 days of hire)
