Position Summary:
The Support Coordinator assists individuals and their families in the development of individualized service plans. This role also assists individuals by identifying and linking needed services and supports to improve the quality of life within the home and the community through annual home visits, quarterly face-to-face contact, and attending interdisciplinary team meetings as necessary. The Support Coordinator maintains a 24-hour on-call system, ensures regular communication with families/guardians or home representatives, and recognizes that the highest quality of service is delivered with Hospitality, Respect, Quality, Spirituality, and Responsibility, the core values of the Agency, and maintains confidentiality of departmental records, persons, and activities.
