Support Coordinator
Position Summary:
The Support Coordinator assists individuals and their families in the development of individualized service plans. This role also assists individuals by identifying and linking needed services and supports to improve the quality of life within the home and the community through annual home visits, quarterly face-to-face contact, and attending interdisciplinary team meetings as necessary. The Support Coordinator maintains a 24-hour on-call system, ensures regular communication with families/guardians or home representatives, and recognizes that the highest quality of service is delivered with Hospitality, Respect, Quality, Spirituality, and Responsibility, the core values of the Agency, and maintains confidentiality of departmental records, persons, and activities.

Essential Duties and Responsibilities:
- Develops a thorough understanding of the services funded by the Division of Developmental Disabilities and ensures these services are utilized in accordance with the SP & CCP Manual.
- Schedules and facilitates planning team meetings in collaboration with the individual and a parent/guardian.
- Monitors and follows up to ensure delivery of quality services and ensures that services are provided in a safe manner, in full consideration of the individual’s rights.
- Maintains a confidential case record that includes but is not limited to the NJ Comprehensive Assessment Tool (NJ CAT), completed SC Monitoring Tools, PCPTs, ISPs, notes/reports, annual satisfaction surveys, and other supporting documents uploaded to the iRecord for individuals served.
- Ensures individuals served are free from abuse, neglect, and exploitation; reports suspected abuse or neglect in accordance with specified procedures and provides follow-up as necessary.
- Conducts contacts monthly, face-to-face visits on a quarterly basis, and in-home face-to-face home visit on an annual basis that includes a review of the ISP and is documented on the SC Monitoring Tool.
- Documents daily activities and interactions via iRecord.
- Participates in planning and implementing cross-training/cross utilization for various department functions.
- Observes Agency and departmental procedures concerning safety, emergencies, behavior management, and professional conduct.
- Monitors client areas for safety and ease of access.
- Participates in all Safeguarding Trainings related to the protection of children and vulnerable adults.
- Participates in departmental meetings, information sharing and in-service training. Offers ideas for improved services and operations.
- Develops professional competencies; keeps abreast of related literature, enhances technical skills and knowledge.
- Promotes teamwork.
- Work flexible hours as needed based on program needs.
- Performs other duties as assigned.
Qualifications for the Position:
- Bachelor’s Degree or higher in any field.
- 1 year of experience working with individuals with intellectual and/or developmental disabilities (I/DD). Experience can include paid employment, volunteer experience, and/or being a family member or caregiver of an individual with an I/DD.
- Successfully complete training required by the Division before rendering services.
- Must have a valid driver’s license and reliable transportation.
- Bilingual (Spanish and/or ASL) a plus.
- Required skills, knowledge, and ability to perform job.

- Ability to learn new tasks quickly.
- Effective oral and written communication skills.
