POSITION SUMMARY: Under supervision of Program Management, utilizing a shared experience, responsible for facilitating, scheduling, and organizing goal achievement planning focused on overall health and wellness to meet the needs of an adult population composed of SMI, dual-diagnosed and/or referred persons. Identifying center paths for members to provide wellness initiatives and supporting management in delivering department objectives. Responsible for ensuring all services are provided consistently with REN’s philosophy and values; performs related duties as required.

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES

• Ensures center advisement is conducted on a regular and ongoing basis with regard to health and wellness services.
• Provides support to members participating in center health and wellness activities.
• Maintains the minimum standard of 65% of worked hours in delivering services.
• Maintains records to include required tracking and reporting.
• Documents progress of each member served and submits billing timely utilizing the REN software program, meeting data validation expectations.
• Works in partnership with program management to enhance programs, promote services and increase membership.
• Meets program management directives and objectives through department strategic planning.
• Participates in clinical staff meetings, trainings, all-staff meetings and other activities as required.
• Submits timely and accurate timecards, mileage and reimbursement requests.
• Completes administrative tasks which may include preparing documents, copying, faxing and making/receiving REN related telephone calls.
• Facilitates groups involving ways to improve total health and overall quality of living. 
• Demonstrates ability to work with people from diverse backgrounds.
• Effectively establishes boundaries and maintains an appropriate, professional work environment with REN members and staff.
• Documents billing notes in regards to group facilitation and one on one encounters with served population. Submits theses notes within a 24 hour time frame from when facilitation occurred.
• Acts as a role model for served population by modeling REN’s philosophy, vision, and values. 
• Teaches others about managing mental and physical health.
• Creates and adapts wellness objectives to overcome barriers to good mental and physical health based on needs of, and with input from clients.
• Provides individualized health coaching to clients in obtaining their health objectives.
• May be required to provide transportation to members on an as-needed basis.

KNOWLEDGE, SKILLS AND ABILITIES REQUIREMENTS 

• Knowledge of the principles of behavioral health recovery and adherence to REN’s philosophy.
• Knowledge of the principles of cultural competency and adherence to its philosophy.
• Knowledge of SMI diagnoses and behavioral health services delivery systems policies and procedures.
• Knowledge of state behavioral health individual and agency licensure regulations and requirements.
• Knowledge of state and local behavioral health resources.
• Ability to work effectively with others and to communicate effectively, orally and in writing.
• Ability to maintain confidentiality of information.
• Ability to work under pressure and address events/crises which may develop unexpectedly.
• Able to type at least 35 wpm
• Basic Microsoft Office experience and ability.

EDUCATION/LICENSE/CERTIFICATION/EXPERIENCE REQUIREMENTS

• The position has a requirement of a shared experience. A shared experience is defined as personally utilizing privately or publically funded behavioral health services, to include substance abuse treatment.
• A High School Diploma/GED.
• Certified Personal Trainer 
• Peer Support Certification (Preferred) 
• Previous service delivery work experience in the behavioral health field. 
• Behavioral Health Technician credentials preferred.
• Some assignments require a current Arizona Driver's License and a current satisfactory 39-month Motor Vehicle Record (MVR).
• Must be 21 years of age.
